If Customer fails to become pregnant after six (6) cycles of tracking with Proov, Customer may send a written request to Proov within 15 days of the first day of the Customer’s seventh cycle and receive a credit to pay for 100% a telemedicine consultation for a reproductive endocrinologist chosen by Company. In order to qualify for this credit, Customer must: (a) initiate the request for the consultation within 15 days of the first date of the menstrual cycle immediately following the sixth consecutive cycle of testing with Proov; (b) provide Company with a copy of Customer’s original sales receipt, credit card statement, or canceled check showing purchases of Company’s products for the preceding six (6) consecutive cycles; (c) provide Company with the PDF “Proov Insight Reports” for the preceding six (6) consecutive cycles; and (d) provide Company with complete contact information for Customer, including address, telephone number and e-mail address (if applicable).
